A couple of years ago I was responsible for a data migration in our billing service. I had tested it on a small dataset, and it worked fine, so I ran it in production on a Friday evening.
It turned out that on the full dataset the migration locked the main payments table, and for about ten minutes customers couldn't pay. I noticed the alerts quickly, rolled the migration back and told my lead and the support team what had happened.
The next week I wrote a postmortem. We introduced two rules: every migration runs on a copy of production data first, and no risky deploys on Fridays. We haven't had a similar incident since then.
What I learned is that a test on a small sample can give you false confidence.
